BEHAVIORAL HEALTH

The Family Health Clinic provides a unique opportunity for working together with your Nurse Practitioner and therapist to meet your mental, physical, and emotional health needs. We provide the following services:

  • Individual and Group Counseling
  • Counseling 
  • Medication Management
  • Stress Management
  • Medication-Assisted Treatment (MAT)

 

What you can expect from counseling:

Counseling is a place where you can discuss concerns you have about your health, your relationships, your job, your school, and other issues that are important to you.  Your therapist will work with you to develop goals and strategies to address the problems you are experiencing.

Although the purpose of counseling is to work toward positive results, the process is not always an easy one, and there are no guarantees.  If at any time you have questions about the services you are receiving, or you want a different type of service, please feel free to discuss those concerns with your therapist.  In addition, there may be times when your therapist does not feel qualified to provide you with the type of services you need and deserve.  In that event, he or she will discuss those issues with you and refer you to an appropriate provider in your community.

 

Scheduling Appointments

Initial intake therapy appointments are usually scheduled for an hour and fifteen minutes.  Most therapy sessions are scheduled for 45 minutes, but we can also schedule 30 minute sessions for children as needed.

The frequency and number of sessions is determined on an individual basis and will be discussed with you by your therapist.  Every effort will be made to schedule appointments at your convenience but is not guaranteed.

 

Family Health Clinic Limitations

Our facility has limitations to the services that we are able to provide to those who are chronically suicidal, homicidal, or Psychotic, personality disorders, and patients who display those symptoms will be referred to a behavioral health entity that is capable of meeting those more intensive needs. 

In addition, patients who are also experiencing issues with substance abuse may need to be referred to an appropriate substance abuse provider as close to the patient’s place of residence as possible or to a substance abuse provider of the patient’s choice.

 

Emergencies

In case of an emergency during business hours, please inform the office personnel of the urgency of your situation so that your therapist or an available Nurse Practitioner can be informed as quickly as possible.

Should an emergency arise after hours, please go to the nearest Emergency Room.

Confidentiality

Matters discussed with the therapeutic relationship are confidential and protected by Indiana State law.  The therapists maintain the highest possible ethical and legal standards regarding privacy and confidentiality.  Your psychological records will be kept confidential in our electronic health records.

There are some instances, however, in which, by law, confidentiality must be broken.  Such instances include, but are not limited to, threats of or suspected danger to yourself or others.  If you become suicidal or homicidal, your family and/or a responsible designee will be contacted to attempt to ensure the safety of yourself or others.  If safety cannot be ensured through contact with family or a responsible designee, law enforcement will be contacted to ensure safety.  At this point, you maybe referred for more intense behavioral health services.  Your therapist is also required by law to report instances of child abuse or neglect and instances of abuse or neglect of individuals who cannot care for themselves, such as the elderly or disabled individuals.

If you desire information to be obtained, released, or exchanged with any other health care professional or individual, your written permission will be necessary.  Appropriate release of information forms will be completed prior to the release of this information.  You should discuss all requests for counseling information with your counselor before singing a release of information form.
