The Family Health Clinic is a community-based, full service primary care medical facility offering a patient centered medical home for the entire family. Everyone is welcome to become a patient at the clinic. The clinic does accept all insurance types, but also welcomes those without health insurance.
The Family Health Clinic offers services for routine medical care, physical exams, disease management, immunizations, laboratory testing and mental health management. The clinic offers high quality, accessible and affordable healthcare for the entire family. The friendly and courteous staff offer compassionate professional care with dignity. Expanded office hours provide easy appointment scheduling in the convenient, community-based facility. With a focus on health education, the Family Health Clinic partners with the patient offering individualized health promotion with a variety of health and wellness programs available.
A twenty-four hour call service allows patients the peace of mind and ability to reach a medical provider after regular business hours.
The Family Health Clinic is not an urgent care facility and does not offer emergency care for chest pain, bleeding, premature labor or trauma. For emergencies call 911 or visit your nearest emergency room.

Founded in 1995 by Purdue University School of Nursing professor Dr. Jo Brooks, the Family Health Clinic of Carroll County was created to provide quality primary healthcare to individuals facing financial or access barriers to care. What began in a small 500-square-foot space in Delphi has grown into a network of four clinic locations serving communities across the region.
Over the years, the clinic expanded to Monon, Burlington, and Wolcott while partnering with Purdue School of Nursing and North Central Nursing Clinics to become a Federally Qualified Health Center. The clinic also earned Patient-Centered Medical Home (PCMH) Level 2 designation and continues to provide hands-on training opportunities for Purdue nursing and pharmacy students.
Today, the Family Health Clinic serves thousands of patients annually through medical and behavioral health services, while also offering programs such as the Breast and Cervical Cancer Program (BCCP) and Medication Assistance Programs (MAP) to improve access to affordable care.

The Family Health Clinic was only located in Delphi Courthouse Square. Dr. Brian Doggett of IU Health serves as the first Collaborative Practice Physician. The first grant was awarded by Indiana Department of Health (IDOH). Patients pay on a cash-only basis.Â

FHCC becomes a part of the provider network for IDOH's Breast and Cervical Cancer Program (BCCP).Â

FHCCC moves locations to Brosman Center (formerly a bank space) off of HWY 421. Application for participation in the IDOH Vaccines for Children (VFC) Program is submitted.Â

After the White County United Way and the Indiana State Department of Health recognized a similar need for access to care, a new clinic site was opened in Monon, Indiana.

Purdue School of Nursing applies for Federal Qualified Health Center (FQHC) status.

American Recovery & Reinvestment Act (ARRA) funds awarded. Trinity location in Lafayette closes. Dr. James Bien joins as collaborating physician for Pediatric providers.Â

Behavioral Health Services were added to the clinics in 2010 as part of an expansion of services.Â

Our Navigators are Licensed Indiana Navigators, SHIP Counselors, and Certified Application Counselors who specialize in Medicaid, Medicare, and Marketplace health insurance. They can answer questions, help with applications, and ensure you understand your coverage options.

In June, NCNC Delphi and Monon clinics receive recognition from the National Committee for Quality Assurance (NCQA) as a Level 2 Patient Centered Medical Home (PCMH). Purdue College of Pharmacy Fellows and Interns begin providing medication management services.Â

Women's Health services added. New building in Monon completed. Medication-Assisted Therapy (MAT) services added with introduction of Vivitrol Program.

The Town of Wolcott and Remington Wolcott Economic and Community Development Corp., along with the REMC, supported opening a fourth clinic site in Wolcott, Indiana.

December 2016: IU Health Foundation donated an office building in Delphi to the clinics, allowing consolidation of administrative functions and opening more exam room space at the Prince William Road facility.

In mid-January, the Family Health Clinic began providing COVID-19 vaccines at all of our locations: Burlington, Delphi, Monon, and Wolcott.

At the Family Health Clinic, we offer a variety of services that help men live a healthy and full life.

We use the Mobile Outreach trailer to store, transport, and distribute essential resources such as toothpaste, toilet paper, laundry detergent, and other hygiene and cleaning products.Â
The Family Health Clinic is a Health Resources and Services Administration health center and was awarded their Health Center Quality Leader Designation in 2019.
HRSA-funded health centers continue to lead the U.S. healthcare system in providing quality, value-based care to their communities. HRSA’s Quality Improvement Awards recognize the work that health centers do to address health priorities by designating health centers that ranked in the top 30% that achieve the best overall clinical performance receive designation as Health Center Quality Leaders.Â
These awards also recognize health center achievements in other areas, including improving cost-efficient care delivery while also increasing quality of care, reducing health disparities, increasing both the number of patients served and patients’ ability to access comprehensive services, advancing the use of health information technology, and delivering patient-centered care.
The patient-centered medical home is a model of care that puts patients at the forefront. PCMHs build better relationships between patients and their clinical care teams.
PCMHs are committed to quality and quality improvement by ongoing engagement in activities such as using evidence-based medicine and clinical decision-support tools to guide shared decision-making with patients and families.
