Getting ready for your first visit to the Family Health Clinic? 

You can start by filling our your new patients forms. Then, head to the Patient Portal and Care Message sections to learn about the helpful services we provide so that you can stay on top of your healthcare easily.

New patient forms

Before your upcoming Family Health Clinic appointment, we encourage you to complete the following form:

In addition to these forms, here are other things to bring to the appointment:

  • Photo ID
  • Birth Certificate for children
  • Your insurance card (if applicable)
  • A month of income documents (if applying for sliding-fee scale)
 
Please note: if you do not have the ability to fill these forms out online, we will have them for you at the clinic.

Patient portal

The Patient Portal will give you, our valued patient, access to your own medical information, which empowers you to take better control of your health and wellness. 

By using the Secure Messaging service included with the Patient Portal, you can now send private messages to your primary care provider at the Family Health Clinics with information or inquiries. In addition to this, you can request appointments through the Patient Portal and even request that an appointment be rescheduled.

At your upcoming appointment, ask any of the staff to help
you register for an account on the Patient Portal. Once
registered, you will always find the Patient Portal at
https://NCNCMyChart.org.

Text messaging

The Family Health Clinic is happy to announce that Text Messaging service will also be available to patients with a mobile device. We recognize that text messaging has increased in use and is a normal way people now communicate. 

For that reason, we have adopted text messaging as a way to keep in touch with you, too. With Text Messaging, we can send you reminders of upcoming appointments. We can inform you of activities you may find important in promoting your health as well as your wellness.

Finally, Text Messaging can be used to remind you of the need to schedule annual exams, pediatric wellness visits, and many other reminders that help you manage your health.
