The Family Health Clinic is a community-based, full service primary care medical facility offering a patient centered medical home for the entire family. Everyone is welcome to become a patient at the clinic. The clinic does accept all insurance types, but also welcomes those without health insurance.

The Family Health Clinic offers services for routine medical care, physical exams, disease management, immunizations, laboratory testing and mental health management. The clinic offers high quality, accessible and affordable healthcare for the entire family. The friendly and courteous staff offer compassionate professional care with dignity. Expanded office hours provide easy appointment scheduling in the convenient, community-based facility. With a focus on health education, the Family Health Clinic partners with the patient offering individualized health promotion with a variety of health and wellness programs available.

A twenty-four hour call service allows patients the peace of mind and ability to reach a medical provider after regular business hours.

The Family Health Clinic is not an urgent care facility and does not offer emergency care for chest pain, bleeding, premature labor or trauma. For emergencies call 911 or visit your nearest emergency room.

Family Health Clinic Opens
Dr. Jo Brooks Opens the Family Health Clinic of Carroll County

The Family Health Clinic was only located in Delphi Courthouse Square. Dr. Brian Doggett of IU Health serves as the first Collaborative Practice Physician. The first grant was awarded by Indiana Department of Health (IDOH). Patients pay on a cash-only basis.

Services Expand
Pediatric Services are Added

FHCC becomes a part of the provider network for IDOH's Breast and Cervical Cancer Program (BCCP).

Family Health Clinic Moves Locations
Family Health Clinic Moves to Brosman Center

FHCCC moves locations to Brosman Center (formerly a bank space) off of HWY 421. Application for participation in the IDOH Vaccines for Children (VFC) Program is submitted.

Family Health Clinic Opens
Handwritten Application to Bill Medicaid is Submitted

Parrett's Meat Processing and Catering hosts an annual fund-raising dinner and event for FHCCC.

Family Health Clinic Expands
FCHHH moves to Prince William Rd.

Purdue School of Nursing applies for Federal Qualified Health Center (FQHC) status.

2009
Family Health Clinic Expands
Family Health Clinics obtain FQHC status

American Recovery & Reinvestment Act (ARRA) funds awarded. Trinity location in Lafayette closes. Dr. James Bien joins as collaborating physician for Pediatric providers.

NCNC hosts first Child Wellness Day on Purdue campus

In June, NCNC Delphi and Monon clinics receive recognition from the National Committee for Quality Assurance (NCQA) as a Level 2 Patient Centered Medical Home (PCMH).

Purdue College of Pharmacy Fellows and Interns begin providing medication management services.

Family Health Clinic Expands
Burlington and Wolcott Locations Open

Women's Health services added. New building in Monon completed. Medication-Assisted Therapy (MAT) services added with introduction of Vivitrol Program.

Operations impacted by COVID-19 Pandemic.

In March, NCNC incorporates telehealth platform into service delivery.

our history

The Family Health Clinic of Carroll County was launched in 1995 by Dr. Jo Brooks, a Purdue University School of Nursing professor, to provide quality primary healthcare to individuals who were unable to obtain care due to financial or access challenges.

The Family Health Clinic staff began seeing patients in a 500-square-foot space on the Delphi Courthouse Square. In 1998, the clinic moved to Brosman Center on Route 421. 

The Family Health Clinic then expanded, opening a satellite location in Monon, Indiana in 2006. In 2008 the Delphi Clinic moved and has been housed at the Brooks Center at 901 Prince William Road, Delphi.

In 2009, a non-profit organization, North Central Nursing Clinics, partnered with Purdue School of Nursing to secure federal funding from HRSA (Health Resources & Services Administration) for the Family Health Clinics of Carroll County and Monon as a Federally Qualified Health Center. 

In June 2014 the Family Health Clinic received the National Committee for Quality Assurance Patient-Centered Medical Home (PCMH) Level 2 Designation. The Clinic also provides training opportunities for more than 30 Purdue School of Nursing students throughout the year as well as Purdue College of Pharmacy Fellows and Interns at each site.

Today, there are four sites. The Family Health Clinic of Burlington opened in January of 2016 and the Family Health Clinic of Wolcott opened in July 2016.

By the end of 2020, we anticipate treating over 8,400 individuals per year during 24,000 visits with a medical or behavioral health provider.  

The Family Health Clinic participates in the Breast and Cervical Cancer Program (BCCP).  They also offer Medication Assistance Programs (MAP) that allow qualified patients to apply to pharmaceutical companies to request prescription medications at little or no cost.

The Family Health Clinic is a Health Resources and Services Administration health center and was awarded their Health Center Quality Leader Designation in 2019.

HRSA-funded health centers continue to lead the U.S. healthcare system in providing quality, value-based care to their communities. HRSA’s Quality Improvement Awards recognize the work that health centers do to address health priorities by designating health centers that ranked in the top 30% that achieve the best overall clinical performance receive designation as Health Center Quality Leaders. 

These awards also recognize health center achievements in other areas, including improving cost-efficient care delivery while also increasing quality of care, reducing health disparities, increasing both the number of patients served and patients’ ability to access comprehensive services, advancing the use of health information technology, and delivering patient-centered care.
