At the Family Health Clinic, a Federally Qualified Health Center (FQHC), we are committed to welcoming every person—regardless of insurance status or ability to pay.
If you’re facing financial challenges, we encourage you not to delay care. Our team will work with you to understand your options, including whether you qualify for our Sliding-Fee Scale, what additional resources may be available, and how to create a payment plan that fits your budget. Referrals to other assistance programs are also available when needed. Your health and well-being always come first.

SLIDING-FEE SCALE
We believe financial barriers should never stand in the way of your health. If you do not have insurance or are experiencing financial hardship, we will work with you to determine eligibility for our Sliding-Fee Scale, review available assistance programs, and set up a manageable payment plan.

Insurance Navigation Services
Our on-site Insurance Navigators are available to help you understand your health coverage options and guide you through the application process from start to finish.
Our Navigators are Licensed Indiana Navigators, SHIP Counselors, and Certified Application Counselors who specialize in Medicaid, Medicare, and Marketplace health insurance. They can answer questions, help with applications, and ensure you understand your coverage options.
Beyond insurance, our Navigators can also connect you with community resources and assistance programs, including help with Supplemental Nutrition Assistance Program (SNAP) applications and other local support services.
Appointments with our Navigators are open to everyone—you do not need to be an established Family Health Clinic patient, and there is no cost for this service.

BArb Hickner
Barb Hickner has over 25 years of experience in human services and community engagement. She holds a Bachelor of Science in Organizational Leadership and Supervision and a Master of Science in Human Services from Purdue University. Her additional certifications as a Community Health Worker and SHIP Counselor allow her to provide expanded services to individuals and families facing life’s challenges in the vast and often misunderstood healthcare landscape. She is committed to addressing the social determinants of health and eliminating barriers to achieving better health outcomes.
Noemi Mendoza
Noemi Mendoza has a strong background in public health and a passion for community-centered care. She holds a Bachelor of Science in Public Health and a Master of Public Health (MPH) in Family and Community Health from Purdue University. She is deeply committed to health equity and believes healthcare is a human right, especially in rural communities where access can be limited.
FAQ’s About Navigation Services
Every situation is different, but in general, please bring the following for all household members applying for coverage:
- Recent proof of income (30 days), such as paystubs or a letter from your employer.
- Tax returns, W-2, 10-99, etc.
- Proof of identity: Driver’s License/photo ID, proof of Indiana Residency
- Birth certificates, passports, etc.
- Social security numbers
- Proof of resources: Bank statements, etc.
There is no cost. Appointments with our Insurance Navigators and Community Health Workers are completely free.
No. Anyone can schedule an appointment with a Navigator—you do not need to be an established patient.
We assist with Medicaid, Medicare, and Marketplace insurance. We can also help guide you to additional resources or next steps for other insurance needs.
The Family Health Clinic’s navigators are able to help with more than insurance. If your income is less than $3,250 per month for a family of four, you may qualify for food assistance. Navigators at the clinic are available to help with the application for SNAP benefits as well as locating additional resources in the community.
